The House Commerce Committee considered a bill that would have forced low nuclear waste sites to stay clear of floodprone areas. The bill would apply to sites that handle radioactive castoffs from medicine and industry, such as rags and clothing. Supporters said they wanted to stop a waste repository 20 miles southwest of Raleigh so radioactivity wouldn't endanger water supplies and people who drink from them. aiming my best to do away with the Wake site.

An attorney advising the committee said the Wake County site has already past that stage and its operators are seeking a license to operate. Autopsies costly for N.C. hospitals State's fee steady as expenses climb Associated Press WINSTON-SALEM Performing autopsies under contract with the state medical examiner's office is a losing proposition for pathologists and hospitals in North Carolina, but the situation isn't likely to change soon. The government pays $400 for each autopsy; usually the money is paid by the county where the person died. Doctors estimate, however, that a typical autopsy costs about $1,000.

"Basically, the hospitals in the state are underwriting the medicalexaminer program to the tune of millions and millions of dollars," says Dr. Patrick Lantz, a forensic pathologist at Baptist Hospital. South Carolina, Virginia and Tennessee pay $800 to $850 for each autopsy. North Carolina's fee is set by state statute and can be changed only by the General Assembly, and legislators haven't changed it in 10 years. When the General Assembly increased the fee from $350 to $400 in 1987, legislators intended to reimburse the doctors and hospitals only for the actual cost of doing the autopsies.

But the cost has climbed steeply in the past 10 years, partly because of new state laws intended to protect medical workers from AIDS and other blood-borne diseases. Lantz now must strap on a surgical face mask every time he steps into his morgue, and he must slip on surgical booties rarely worn 10 years ago before every autopsy. About 60 contractors perform autopsies for the state. Most of the doctors involved are pathologists who handle a handful of cases each year. Several are certified forensic pathologists with special training in toxicology, gunshot wounds and trauma injuries.

"They have been pressing us to somehow subsidize their fee," said Michael Olson, the administrative services manager for the office of the chief medical examiner. "There is nothing we can do. Nothing." Pathologists perform about 1,800 autopsies a year for the medical examiner's program about half of the total. The chief medical examiner's offices in Chapel Hill and Charlotte handle the rest. Lantz handles about 200 autopsies a year, usually on people who died as a result of crime or under suspicious circ*mstances.

The cause of death is often apparent a gunshot wound, crushed organs or disease. Sometimes the cause is elusive. Baptist is the largest of the private contractors in the state. An average of 400 autopsies a year, on bodies from a 16-county region, are done by two doctors who split their time between teaching and medicine. The loss from autopsies totals about $240,000 a year, Lantz said.

Baptist and other pathology centers, including the hospital at East Carolina University, have lobbied the state medical examiner's office to increase the fee, but to no avail. Dr. John Butts, the state's chief medical examiner, suggested this year that the fee be doubled. No increase was included in the budget proposal that Gov. Jim Hunt presented to the General Assembly.

Several pathologists have dropped out of the medical examiner's program in the past five years, said Olson. Those departures have shifted some of the burden to places such as Baptist and ECU, said Lantz and Dr. Mary Gilliland, head of pathology at ECU. Baptist and ECU both operate their pathology departments with one less doctor than they had just a year ago. Gilliland, the only doctor in ECU's pathology department, said she will lower the number of autopsies she conducts from about 350 to 250 next year.

Meanwhile, Baptist is considering another idea to make up for the shortfall charging a consultation fee to prosecutors and defense attorneys, who currently pore over autopsy results with the doctors at no charge.
